Remote Patient Monitoring

Remote Patient Monitoring
What Is RPM

Track Patient Vitals From Home — In Near-Real Time

Remote Patient Monitoring lets patients take vital sign readings at home using connected Bluetooth medical devices. Each reading is transmitted securely to the Zelicare platform, where clinicians can review it, set personalized thresholds, and receive automatic alerts when something needs attention — all without requiring an office visit.

How It Works

From Reading to Clinical Action — In Four Steps

1
Patient Takes a Reading

The patient uses a paired Bluetooth device at home — blood pressure cuff, pulse oximeter, or weight scale — and the Zeli Care mobile app captures the measurement.

2
Data Transmitted Securely

The reading is sent from the phone to the Zelicare server over an encrypted TLS connection. It appears on the care team's dashboard within moments.

3
Evaluated Against Ranges

The platform checks the reading against the clinician-defined safe range for that patient and vital type. If it falls outside the threshold, an alert is created.

4
Care Team Acts

The assigned nurse receives an in-app notification. They review the alert, acknowledge it, and take action — or forward it to the doctor. Critical alerts auto-escalate if unacknowledged.

Medical Devices

Connected Bluetooth Devices Tested & Proven

Zelicare is engineered and clinically tested with A&D Medical Bluetooth devices. Each device communicates using specific BLE protocols that the platform understands — ensuring accurate, reliable data capture every time.

Blood Pressure Monitor

A&D UA-651BLE — measures systolic and diastolic blood pressure plus pulse rate. Pairs via Bluetooth; supports automatic background scanning on Android for hands-free reading capture.

Pulse Oximeter

A&D TM-200BLE — measures blood oxygen saturation (SpO2) and pulse rate. Clips onto the fingertip; readings are captured with a single tap in the mobile app.

Weight Scale

A&D UC-352BLE — measures body weight in kilograms or pounds. Step on the scale, press Set, and the app captures the reading. Clinicians can configure weight-change alerts over time.

Why these devices? A&D Medical devices follow the Bluetooth SIG Blood Pressure and Weight Scale service standards and have been validated in clinical settings. The Zelicare mobile app implements their specific BLE bonding and discovery protocols for reliable pairing on both Android and iOS.

Platform Capabilities

Everything Your Care Team Needs — In One Place

Clinical Dashboard

Role-based dashboards for nurses, doctors, and administrators — each sees exactly the patients and data relevant to their role and organization.

Smart Alert Routing

Alerts are owned and routed — nurse-first by default, with automatic escalation to doctors. Forwarding, acknowledgment, and resolution tracked end-to-end.

Per-Patient Ranges

Clinicians set custom safe ranges per patient, per vital type. System-governed hard safety thresholds (like SpO2 below 90%) cannot be relaxed.

Secure Messaging

Patients can send secure text messages to their assigned nurse. Notifications appear in-app — no PHI exposed outside the platform. Not for emergencies.

Video Consultations

Built-in video visits let care teams connect face-to-face with patients — no separate app needed. Session metadata is logged for compliance.

Nurse–Patient Assignment

Organization admins assign patients to nurses. Each nurse sees only their assigned caseload — data, alerts, and messages are scoped accordingly.

Audit Trail

Every login, data access, range change, and alert action is recorded. Administrators can filter, search, and export the full log for compliance review.

Multi-Org & Solo Practice

Supports multi-provider clinics, nursing homes, home health agencies, and solo practitioners — each with configurable capacity and role structure.

Who Benefits

Designed for Every Role in the Care Team

Patients

Take readings at home with simple Bluetooth devices. View your own vitals, out-of-range history, and message your nurse — all from your phone or browser.

Nurses

Monitor your assigned caseload in real time. Receive instant alerts for out-of-range readings, respond to patient messages, and forward critical cases to doctors.

Doctors

Review patient data across your organization. Act on escalated and forwarded alerts. Set per-patient clinical ranges. Maintain oversight without replacing the nurse workflow.
